Have you packed your hospital bag yet? Aside from baby things and mommy essentials, be sure that you have prepared the following documents that you must bring with you to the hospital.
Yup! Even when you are in labor, paperwork does not stop. The hospital will issue the certificate of live birth, also known as birth certificate. Hence there will be some requirements that you must prepare in advance. Also, some documents will help nurses or doctors such as the baby record.
1. Marriage Certificate
2. Valid IDs – you and your husband’s
3. OB-GYNE’s referral letter
4. SSS and PhilHealth documents (if you are an active member entitled to benefits)
5. Latest ultrasound results, test results, baby record
